Patient Care Coordinator

Learn how to advocate for patients’ holistic care and coordinate the services they need as a Patient Care Coordinator. Through our Patient Care Coordinator program, you will become an excellent communicator and educator, with the skills to influence behavior changes that lead to increased wellness and healthier lifestyles. You will gain a better understanding of the root causes of patients’ medical needs, and reflect on various social determinants and medical policies to develop realistic care plans. With the support of knowledgeable program faculty and staff, you will complete the Patient Care Coordinator program with the skills you need to work in administrative and clinical settings to provide ethical patient care.
